Elder care

Elder Care at Home

Blood pressure and blood sugar checks for grandparents, and home visits for those who cannot walk to us.

A nurse in gloves bends over a desk to write a record while a colleague stands beside her

About this programme

High blood pressure has no symptoms until it causes a stroke. Many grandparents in Rakai have never had it measured, and the ones who have often cannot afford the trip to collect their tablets each month.

At every outreach day we keep a bench for elders, with a nurse, a blood pressure machine and a glucometer. We explain the numbers in plain Luganda, write them in a small card the person keeps, and link anyone with high readings to the health centre for ongoing care.

For jjajjas who can no longer walk far, our Centers keep a list and arrange home visits. Often the visit is as much about company as medicine, and we think that matters too.

What we do

Inside this programme

  1. Blood pressure checks

Measured twice, recorded on a card the person keeps, explained in plain language.

  2. Blood sugar checks

Glucometer checks for elders with risk signs, with advice on food and follow up.

  3. Home visits

Centers keep a list of elders who cannot travel and visit them at home.

  4. Medicine reviews

We check what tablets people are taking, and whether they are taking them.

What we count

  • Elders screened for blood pressure
  • Elders linked to ongoing care
  • Home visits made
